Background: Smokers who continue to smoke up to the point of surgery are at increased risk of a range of complications during and following surgery. Study selection: Studies testing the effect of smoking reduction interventions delivered at least 24 h before elective surgery were included. Study appraisal and synthesis: Potential studies were independently screened by two people. Data relating to study characteristics and risk of bias were extracted. The effects of the interventions on pre-operative smoking abstinence were estimated using random effects meta-analyses. The association between specific intervention components behavior change techniques; mode; duration; number of sessions; interventionist and smoking cessation effect sizes were estimated using meta-regressions.

Smoking cessation , usually called quitting smoking or stopping smoking , is the process of discontinuing tobacco smoking. Tobacco cessation significantly reduces the risk of dying from tobacco-related diseases such as coronary heart disease , chronic obstructive pulmonary disease COPD , [5] and lung cancer. Many strategies can be used for smoking cessation, including abruptly quitting without assistance " cold turkey " , cutting down then quitting, behavioral counseling, and medications such as bupropion , cytisine , nicotine replacement therapy , or varenicline. Most smokers who try to quit do so without assistance. In nicotine-dependent smokers, quitting smoking can lead to symptoms of nicotine withdrawal such as nicotine cravings, anxiety , irritability, depression , and weight gain. It is common for ex-smokers to have made a number of attempts often using different approaches on each occasion to stop smoking before achieving long-term abstinence. We read with interest the work of Turan et al. The accompanying editorial by Katznelson and Beattie 2 provides a valuable perspective on their work, and we applaud their call for anesthesiologists to take a leadership role in efforts to help surgical patients quit smoking. In addition to potential beneficial effects on the acute perioperative risk nicely documented by Turan et al. The issue of the optimal timing of preoperative smoking cessation is of practical importance, and the duration of preoperative abstinence necessary for maximum benefit is not defined and may differ among various smoking-related complications. For example, recent data suggest that even prolonging postoperative abstinence in smokers who had smoked up to the time of their surgery may benefit patients who have undergone orthopedic surgery.

The literature was reviewed to determine the risks or benefits of short-term less than four weeks smoking cessation on postoperative complications and to derive the minimum duration of preoperative abstinence from smoking required to reduce such complications in adult surgical patients. Using a random effects model, meta-analyses were conducted to compare the relative risks of complications in ex-smokers with varying intervals of smoking cessation vs the risks in current smokers. We included 25 studies.
